RAFFLE MECHANICS:

  1. Raffle period is from September 16 – November 10, 2019
  2. This raffle is open to all InLife Health Care members who are:
  • Philippine citizen or permanent resident of the Philippines;
  • Of legal age, 18 years old and above;
  • Enrolled under any of the listed InLife Health Care plans: InHealth Corporate, InHealthBiz, InHealth Persona, InHealth Prepaid.
  • Not employees or accredited sales agents of InLife Health Care, their relatives up to the second degree of consanguinity or affinity and any other party who are directly involved in organizing, promoting or conducting the draw.
  1. To be eligible for the raffle draw, the InLife Health Care member must complete steps:
    • Download InLife Health InLife Health mobile app found at Google Play Store (http://bit.ly/InLifeHealthAndroid) or Apple App Store (http://bit.ly/InLifeHealthiOS) during the raffle period
    • Sign up on the app and complete member profile (email address, password, member ID, birth date, and mobile number).
    • You will receive an email notification to notify you that you have earned one (1) raffle entry.

Keep the downloaded app on his mobile device

  1. Deadline to earn a raffle entry for the weekly draw is until every Sunday at 11:59PM.
  2. The promo organizer’s decision with regard to the raffle mechanics, winners and prizes, with the concurrence of DTI, is final.

  1. There will be 5 winners of P200 worth of Sodexo gift certificate per week, to be drawn electronically on http://www.randomresult.com/num.php every Monday starting on September 23.
  2. All non-winning entries for the week will remain eligible for the next weekly draw.
  3. A member can only win once within the duration of the promo
  4. Raffle draw will be held at InLife Health Care’s head office at 2F Insular Health Care Building 167 Dela Rosa cor. Legazpi St. Legazpi Village, Makati City, in the presence of representatives from the Department of Trade and Industry (DTI).
  5. InLife Health Care will shoulder the winner’s tax relating to the raffle prize. The raffle prize may not be converted to cash.
  6. InLife Health Care reserves the right to verify the eligibility of the winning entry.

 

CLAIMING AND NOTIFICATION

  1. Raffle draw winner will be notified by InLife Health Care through registered mail.
  2. To claim his/her prize, winner must reply to notification registered mail with the following items:
  • One (1)valid government-issued I.D. (SSS, Unified Multi-Purpose ID, Driver’s License, Voter’s ID),
  • A screenshot of his profile on the InLife Health app

Claiming of the prize must be done within 60 days from when the notification registered mail is received.

  1. The prize Php 200 worth of gift certificate will be sent to the winner via SMS.
